The Integrated Home Care Service is a service that aims to assist and contribute to improving the quality of life of frail people at home. In fact, one's home is the ideal environment to preserve and stimulate the residual capacity for autonomy thanks to the presence of the network of relatives, friends and caregivers who usually surround the assisted person in normal daily life and direct integration with the territory. The ADI recognizes the figures close to the assisted person (relatives, friends, caregivers) as essential figures for obtaining the best result.

THE GOALS   OF INTEGRATED HOME CARE ARE:

  • - Services for people who are partially, temporarily or totally non-self-sufficient
  • - Services for people living with HIV and AIDS
  • - Services for terminally ill and non-terminal cancer patients
  • - Services for people with mental disorders
  • - Services for people in a state of dependence
  • - Home rehabilitation treatments for people with physical, mental and sensory or mixed disabilities

What the service includes

The service consists in the implementation of an Individual Assistance Plan (PAI) developed by the personnel in charge of the Local Health Authorities and shared by the assisted person and the family. This PAI, sent to Fisiodem Cooperativa Sociale, will be provided with the professional figures involved, who will access the home coordinated by the staff of the Operations Center in order to provide a punctual, efficient and effective service. Fisiodem Cooperativa Sociale avails itself of the collaboration of professionals with proven experience in the management of Integrated Home Care.

Who is the service for?

Integrated Home Care is aimed only at people:

  • totally or partially non-self-sufficient, for a period of time established by the ASL and re-evaluated over time
  • unable to walk or be transported to outpatient health facilities;
  • supported daily by a caregiver;
  • in housing conditions suitable for assistance. The goal of integrated home care is to provide an important response to the needs of users, limit functional decline and improve the quality of daily life by allowing patients to reach and maintain the highest possible level of well-being and health.

ADI Service Organization

The home team, which does not replace the family in its tasks, is composed of doctors, nurses, OSS, rehabilitation professionals (physiotherapist, psychomotor therapist, neuro and psychomotor therapist of developmental age, occupational therapist, speech therapist), dietitian, psychologist, social worker. Home care, to be effective, requires that the family is able to organize itself in order to guarantee a constant and active presence.

How the service works

The service operates through:

  • scheduled home visits by healthcare and social healthcare personnel
  • prompt availability of the doctor and nurse (for level II-III patients).

Home visits are organized according to the needs of the individual patient and based on what is established in the Individual Assistance Plan (PAI) drawn up by the relevant ASL District.

The Care Team

Fisiodem Cooperativa Sociale is able to offer home care services with low, medium and high intensity levels.

The service involves an operational staff composed of the following professional figures:

  • Central operations coordinator doctor
  • Nursing coordinators operations center
  • Nurses
  • Rehabilitation professionals
  • Social Care Assistants / Social Health Workers
  • Social workers
  • Psychologists
  • Dietitians

The facility is supported by specialist doctors to provide consultations. The team uses the Operations Centre, whose functions include:

  • telephone support for users;
  • telephone support to operators and the assistance network.

Fisiodem Cooperativa Sociale home care workers will be recognizable by an identification badge, which they will show upon request of the patient or his delegate.

The Health Record

This documentation makes up the patient's home health record, which includes:

The service involves an operational staff composed of the following professional figures:

  • the Service Charter
  • the PAI
  • the Clinical Diary
  • the Information pursuant to EU Regulation 679/2016 and the declaration of consent to treatment
  • the form for reporting complaints and commendations
  • the user satisfaction survey module.
  • Useful telephone numbers for any eventuality

The PAI, or Individual Care Plan, is the summary document drawn up by the Multidisciplinary Evaluation Unit of the ASL, which collects and describes the information relating to the patient's care needs. It contains elements of clinical, care, social, psychological evaluation, integrated and shared with the family unit and/or with the caregiver of the assisted person. The Home Clinical Diary is the fundamental tool for the communication and integration of all the activities carried out by the various operators at home and allows the patient's care path to be retraced.

Operator identifiability All staff are provided with a specific identification document, which is kept in evidence.
Clearly and correctly inform the patient about the facility's services The reception staff is able to provide detailed information on the services that FISIODEM offers, the methods of delivery, access times and costs of individual services.
Respect the patient's dignity and privacy The patient's personal data is stored in the FISIODEM computer archive and can only be accessed by authorized personnel and for justified reasons. The healthcare personnel is bound by professional secrecy. The clinics and physical therapy boxes are kept constantly closed when the service is provided. The toilets are equipped for the disabled.
Ensure the professionalism of your operators

FISIODEM ensures

  • contacts and professional exchanges between colleagues inside and/or outside the facility in the diagnostic-therapeutic process of the user
  • the constant updating of the staff
  • the courtesy and availability of the staff towards users
Ensure appropriate and constantly verified technologies FISIODEM periodically carries out checks on the functionality of the equipment with the support of qualified technicians. The Health Management and the Administration are continuously updated on the new technologies that the market offers and evaluate the acquisition opportunities.

The health management ensures that the activities are carried out by qualified personnel who are continuously trained for such services and guarantees their insurance coverage.

From 09.00 to 20.00 from Monday to Friday - from 09.00 to 13.00 on Saturday..

The calendar of specialist visits is available on site.

The ADI Operations Centre is guaranteed to be open for 6 days from 8:00 to 18:00 from Monday to Friday for a minimum of 6 hours and from 9:00 to 13:00 on Saturday.

The Secretariat is open from 8.00 to 20.00 from Monday to Friday and from 09.00 to 13.00 on Saturday.

Any interruptions, closures or changes in the opening hours of the service will be communicated promptly to users.
